This post involves providing a commissioned service as agreed with the NHS at the point of discharge for individuals living in their own homes. It will involve a variety of different task which may include, collection of patients from hospital, helping them to get to appointments, basic food preparation, light cleaning, shopping and collecting prescriptions for them. Successful individuals will be compassionate, organised and highly motivated to provide high levels of support to clients, ensuring that those initial weeks post discharge pave the way to returning to living independently at home.

A post of this nature is subject to an enhanced Disclosure Check through the Disclosure and Barring Service.

The post is part time, initially 20 hours per week on a 12 month contract, based on the needs of the service. Flexibility is required Monday – Friday between the hours of 9am and 4pm.

The pay rate is £10.07 per hour.  Holiday is 20 days plus bank holidays, rising by an additional day with each year of service up to a maximum of 5.  Occupational sick pay is available after successful completion of the probationary period.
